what certs are minimally required for registering modem and getting online in D3?

the default certificate format is .cer or .der?

.cer. if you have .der you can use certutil to convert it. Are you trying to (re?)generate certs yourself?

no, not self-signed. Certs are in hex-dump form, need convert them to .cer.

I used MS Certutil, it works for me.
